CareClinic integrates with Apple Health and Google Fit, which can receive data from many Bluetooth glucose meters.
Check if your meter syncs with Apple Health/Google Fit first, then enable glucose data in CareClinic's integration settings. For non-connected meters, use the quick-entry feature to manually log readings with timestamps.
